Kate Talbert is a Master’s prepared Registered Nurse with ten years of experience creating, leading and sustaining innovative community health programs. In her current role, Kate leads the Innovation 2 (INN2) Project for Westside Infant Family Network: a 4-year $16 million contract with the LA County Department of Mental Health designed to build a trauma-resilient community in West and South LA. The project takes a multisector, multi-stakeholder approach to preventing intergenerational trauma in families with young children. In her last role, Kate was the Director of Baltimore’s Accountable Health Community (AHC) project: a five-year, $4.3 million Cooperative Agreement with the Centers for Medicare and Medicaid Services (CMS). Under the AHC project, Kate convened Baltimore’s major health systems and community-based organizations with the goal of screening patients for social needs, improving connections to social resources, and driving systems change to improve health outcomes. Prior to her work with Baltimore, Kate led several community-based interventions delivering care management, chronic disease screening and education for a hospital in Howard County, Maryland.
